Memo and Questionnaire Assignment: 5% of final grade
Required Revision Assignment

Goals Collecting primary data to support your report is a major emphasis of WRIT 465, and the questionnaire- writing process will be explained in detail to you, with handouts.
What You Turn In

For this assignment you must turn in hard copy in your manila envelope:

1. A memo (formatted according to the textbook) addressed to me that explains

  • whom you have chosen to survey and why,
  • how many questionnaires you will distribute,
  • and how and when you will distribute and retrieve the questionnaires. Note: If you plan to give out your questionnaires in a public place like a campus building or a mall, please indicate how you have obtained permission to do so.

This memo need be no longer than a paragraph or two in length but it must contain all the required information or your grade for the assignment will be "F".

2. A copy of your questionnaire, formatted just as you intend to distribute it. At least seven questions are expected. Your questions should be designed to elicit valuable information from the respondents that will help you develop your big report. It will be evaluated on

  • the choice of questions (both for strategy and question design)
  • the clarity with which instructions are given
  • the sequence and strategy in which questions are asked
  • the overall visual impact (layout on the page, typography, use of white space, etc.)
  • the execution
Piloting Questions

Bring five copies of your questionnaire to class on the assigned date. The best way to test questions for accuracy is to "pilot" them, and we will be your pilot audience. Please note the penalties described on your syllabus for failing to bring in a rough draft on the required due date.
Grading

The assignment will be grading according to the Rubric for Writing 465.
